One-on-One Rx is a service designed to provide enhanced care management to all individuals covered by any Atrium Health Wake Forest Baptist health plan. At no cost, participants are offered comprehensive medication management, health navigation, and care coordination from a team of pharmacists, nurse navigators, and pharmacy technicians. All care is coordinated in collaboration with the participant’s primary and/or specialty providers. Proactive and routine outreach may occur through telehealth visits (audio/video) or e-visits (MyAtriumHealth). At this time, all beneficiaries are automatically enrolled in this service and may be contacted by a team member. Our Services Include:
- Individualized assessment of medication regimens.
- Education and counseling on medications.
- Collaboration and communication with the primary care or specialty provider for recommendations on therapeutic and/or cost-effective medication alternatives.

How to Request an Appointment
You can request your One-on-One Rx appointment online. Appointments are available by phone or virtual visit (video or MyAtriumHealth). If needed, in-person visits may be scheduled at the Pharmacy Care Clinic.
Things To Know About Your One-on-One Rx Appointment
- Appointments may be completed by either a phone call, video visit or message through MyAtriumHealth. A face-to-face meeting may be available at the Pharmacy Care Clinic.
- Dependents who are 18 years and older wishing to conduct a One-on-One Rx consultation must schedule their own appointment to meet HIPAA guidelines.
- Some teammates may already be on the most optimal and cost-effective regimens, therefore, there may not be any further recommendations.

With a comprehensive knowledge of medications and care coordination, this team can provide recommendations in alignment with the health plan.
All teammates and dependents enrolled in any Atrium Health Wake Forest Baptist health plan.
Appointments may be completed by either a phone call, a virtual visit or a face-to-face meeting.
Appointments are an informal way to have a conversation with a pharmacist, pharmacy technician or ambulatory nurse navigator about your medications and overall health. The pharmacist will review your prescriptions and over-the-counter medications for both therapeutic and cost effectiveness. If you have any concerns, those topics will be covered in depth as well.
Please have your list of prescriptions, over-the-counter medications, and herbal supplements prepared, in addition to any questions or concerns.
The legal guardian of any dependent under 18 years old is able to discuss the dependent’s health and medication(s) during the guardian’s appointment. Dependents 18 years and over must schedule their own One-on-One Rx appointment to discuss their medications and comply with HIPAA guidelines.
No. One-on-One Rx is available year-round to assist teammates. If you have new questions/concerns or new medications you would like to discuss, feel free to contact us.

Yes. The information discussed during your appointment will be documented in your medical record and will not be discussed with anyone else unless requested by you or necessary to collaborate with your provider(s).

Your One-on-One Rx encounter will be documented in your medical record so other healthcare providers that have access to your record are able to view the encounter. One-on-One Rx can communicate directly with your provider(s) after your visit if an intervention is deemed necessary (e.g. medication change request).
Not all requests may result in appointments, if deemed unnecessary. The question may be delegated to another team if appropriate (e.g. outpatient pharmacy, primary care physician). Some requests may be able to be answered via email/MyAtriumHealth message.
